Registration

ONLY complete this form after you have registered with the MS Society as a 2018 BPMS150 participant. Please read through this registration form carefully. There is a non-refundable registration fee to join Team Texas Children's. 

Agreement:

As a member of Team Texas Children's, I agree to the following:

1. I understand that as a member of Team Texas Children's, I am expected to be a safe, courteous cyclist and a good ambassador for Texas Children's Hospital in the community.  Failure to do so may result in my removal from the team.

2. I understand that as a team member, I am required to volunteer for at least one Team Texas Children's or Ready2Roll Cycling Training Series event during the training season.

3. I understand that I am required to complete the online Safe Cycling Challenge offered by the MS Society.  My 2018 team jersey will be withheld until the quiz has been completed.

4. I understand that I am required to complete the League of American Bicyclist 3-hour bike safety class before the 2018 BP MS 150. http://www.bikeleague.org. This requirement will be waived if the class has been taken in the last three years. Proof must be provided. My 2018 team jersey may be withheld if the course is not completed.

5. I understand that I am required to raise a minimum of $500 for the MS Society. If I don't raise this amount, I may not be not be invited to ride with the team again in the future.

I also understand that the MS Society requires me to raise at least $400 prior to the 2018 ride, and that I will not be allowed to participate in the ride weekend if I do not raise this amount.
